Structured computerized assessment and clinical interview to measure attention, impulsivity, and activity levels. Required for all new patients being evaluated for ADHD.

Diagnosis and treatment decisions are clinician-determined and not guaranteed.

A superbill can be provided for possible out-of-network reimbursement.

  • Missed or late-cancelled appointments fee (less than 48 business hours’ notice): Initial Evaluation – $150 | Follow-Up Appointment – $75
  • Administrative Forms & Letters: $50–$100 depending on complexity. Form completion fees are not billable to insurance and are the patient’s responsibility.
  • Medical Record Copies: $20.62+ per page fees; patients are responsible for associated postage costs.
  • Payment is due at the time of request. Rates are subject to change. Self-pay patients will receive a Good Faith Estimate upon request in compliance with the No Surprises Act.

Yes. We provide structured ADHD evaluations that include clinical interviews and objective computerized testing to assess attention and focus. For individuals with more complex presentations—such as coexisting learning differences or autism—we recommend a full neuropsychological evaluation for a comprehensive assessment.

Yes. You’ll meet with the same psychiatric provider throughout your care for consistency and trust.
